The Centre for Rehabilitation and Health (CRH) was established in 2008 by Tamra Ellis, an Occupational Therapist, with the aim of providing high-quality rehabilitation services across Canada. CRH is a leading provider of rehabilitation services, offering national coverage and a wide variety of services, including full case management, account management, Initial Assessments, Micropoint Simulation (MPS) cognitive behavioural therapy (CBT), and the Progressive Goal Attainment Program (PGAP).

Our Services

CRH is committed to staying at the forefront of the latest training and skills upgrading in a wide range of service needs. This allows CRH to deliver a broad scope of interventions through the same clinician, ensuring continuity of care, cost-effectiveness, and the opportunity for multidisciplinary service options at the cost of a single discipline provider.

The Centre for Rehabilitation and Health utilizes the Progressive Goal Attainment Program (PGAP), an up to 10-week, evidence-based, standardized life role reintegration program that focuses on reducing disability despite the presence of persistent physical and/or mental health symptoms. CRH maintains a network of over 150 PGAP-trained clinicians across Canada, and referrals received through the Central Referral Service will be distributed to PGAP providers who are part of the PGAP Provider Network.

CRH offers a multi-disciplinary scope of services, including occupational therapy, physiotherapy, kinesiology, and social work, but with a single provider, at a fraction of the cost of multi-disciplinary programs. CRH clinicians are passionate about providing support to their clients to assist them to return to meaningful, productive lives with occupational goals.

Our Team

CRH maintains a network of highly trained health care professionals across Canada. Our team includes Tamra Ellis, the founder of CRH, who has over 17 years of experience working with clients with chronic pain and/or mental health symptoms. She has devoted her career to identifying the best evidence-based treatment programs to deliver to her clients and through the team of clinicians she oversees across Canada. Our team also includes Leanne Conick, an account manager, who graduated from the University of Western Ontario with an Honours Bachelor of Health Sciences degree.

In conclusion, CRH is the national referral service for PGAP. We are committed to providing high-quality, cost-effective services that are provided by one clinician in an opportune and efficient response time.
