Born out of the April 2017 merger of the Anne Johnston Community Health Station and Tobias House Attendant Care Inc., our organization was formed to create a dynamic and more client-centric approach to delivering high-quality health and social care programs.

Our mission is to empower independence, health, and well-being together, and we do this by providing a range of innovative health and social care programs that cater to the diverse needs of our clients. Our programs are designed to promote health, prevent illness, and provide support to individuals who require attendant care and personal support. We believe that everyone deserves access to quality healthcare, and we strive to make this a reality for our clients.
Our Services
We offer a comprehensive range of services, including primary healthcare services, attendant care and personal support, health promotion and prevention programs, and community development initiatives. Our primary healthcare services include health check-ups, chronic disease management, and mental health support. Our attendant care and personal support services provide assistance with daily living activities, such as personal care, meal preparation, and light housekeeping. Our health promotion and prevention programs focus on harm reduction, gender-based violence prevention, and diabetes management. We also offer community development initiatives that promote social cohesion and community engagement.
